Payment Options

At My Family Vision Clinic, our mission is to provide quality service and products at reasonable pricing. We accept the following payment methods:

  • Cash
  • Check
  • Visa and MasterCard
  • Vision Service Plan(VSP)
  • Medicare
  • Care Credit

Patients are responsible for payment of services My Family Vision Clinic has provided to them. We are Medicare participating providers which means we accept payment on any fees for services Medicare approves once your deductible has been met. The balance of approved charges not met by your supplemental insurance is your responsibility. We accept Nebraska Medicaid patients.

We will assist you in billing any insurance plan you may have that covers vision care. Our policy on ordering materials is a deposit of 1/2 down to start the order. The balance is due on delivery of the product.

Insurance

Medicare
My Family Vision Clinic participates in Medicare. This means we accept payment in full for all fees that Medicare allows once your deductible has been met. For instance, the eye exam is seen as two parts by Medicare – the first and larger portion is for the eye health exam. The second is for the refraction (this is the determination of the lens power you need in your glasses or contacts). The refraction is a non-covered service. Medicare will pay for a portion of the first pair of glasses following cataract surgery. Deluxe frames, lens tints and coating are not covered by Medicare and are the patient’s responsibility. This is for your information only. My Family Vision Clinic will calculate your Medicare Benefits and the balance that is your responsibility. We will file the claim at no charge to you.

(VSP) Vision Service Plan
This is a common vision plan in our area. It is your responsibility to inform us if you have VSP. The benefits vary depending on the specific plan your employer has chosen. Generally VSP will cover your eye health and vision exam except for a small co-pay. The eyewear portions of your benefit will pay for the majority of the cost of your glasses or contacts. As a VSP paid clinic, we will calculate any out of pocket expenses for you. These are due at the time of service. VSP does not restrict you in you choice of frames and lens options. VSP allows you to choose eyewear or contact lenses you desire while enjoying substantial savings from the normal cost.

Medicaid, Kid’s Connection and Head Start
My Family Vision Clinic accepts Nebraska Medicaid, Kids Connection and Head Start patients. Some Medicaid patients have a small copay that is due at the time of service. We carry a nice selection of frames for all ages of Medicaid patients. All medicaid patients must present their new cards before we can receive authorizations. Since July 2012 the old type of Medicaid cards are no longer valid.

Other Insurance
Other insurance plans accepted are: Eyemed, Vision Care Direct, CEBT, and Tricare and Migrant Health. Safety eyewear plans are accepted from: Burlington, NPPD, Trenton Agriproducts and State of Nebraska.

Patients are responsible to bring proper paper work with them to their appointment.
